Repair Information

- All repairs are done by certified OTC DAIHEN service technicians
- Contact our service coordinator for any further details for repair
- Details are important! Please be prepared to provide error codes, symptoms of failure and serials numbers, as this information will help us determine the root cause of the failure.
- Return Authorization (RA) forms can be downloaded via this page.
- An RMA# must be obtained prior to shipping any equipment in for repair.
Confirming if Repair is Required
If you are experiencing difficulties with your equipment, we strongly urge you to first contact our service department. A service technician can help you verify the problem and can guide you through troubleshooting. For more information on our service support structure, please see our service support page.
Returning Equipment
Upon confirmation that repair service is required, please submit a Repair Authorization (RA) form. The RA form serves as a request to receive a Return Material Authorization (RMA) number and allows OTC DAIHEN to receive proper payment information before the repair occurs.
Note: An RMA# must be obtained prior to shipping any equipment in for repair.

Return Authorization (RA) Forms
For welding power supplies other than the CPDPAS-350 and CPDPAS-501 models, please fill out the RA form for our Charlotte repair center.
For all other repairs, including teach pendants, interfaces, etc., please fill out the Repair Request Authorization form for the Tipp City repair center.
